CT Money Follows The Person Program

The MFP program is a nationally featured demonstration program for rebalancing Connecticut’s long-term care system.  Individuals under the program are afforded the opportunity to have maximum independence and freedom of choice about where they live and receive services.

There is a broad array of services available under the program.  Transition Coordinators provide one-to-one assistance with coordinating the move from facility to community.  There is also assistance with directing community supports, housing, accessibility, and coordination to ensure a smooth transition.  Six different service and support options are available to meet the individual needs of those who transition to the community. 

As the assigned Fiscal Agent, Allied Community Resources is responsible for:

  • Providing in-home enrollment, training, and information to program participants who choose to self-direct their services as a household employer.
  • Verification of Provider credentials and employment documentation
  • Maintenance of the statewide Provider Directory
  • Process Timesheets and Invoices on behalf of the program participant using program funds
  • Conduct statewide outreach to prospective providers

Each participant and or employer is assigned a direct customer representative committed to providing the highest level of customer service.    Our staff has an extensive working relationship with Social Workers, Care Managers, Transitional Coordinators, administrative staff, service coordinators, providers and employers.
