Connecticut’s “Money
Follows the Person” (MFP) program is a $56
million demonstration that is intended to assist
with rebalancing Connecticut’s long–term care system
so that individuals have the maximum independence
and freedom of choice about where they live and
receive services.
There is a broad range of services available
under “Money Follows the Person” (MFP). Transition
Coordinators provide one-to-one assistance with
coordinating the move from facility to the
community. They also assist with directing community
supports, navigating the systems, and accessing
appropriate resources. If people transitioning need
housing, Housing Coordinators assist by locating
acceptable apartments or other suitable living
arrangements.
Some funds may be available to assist individuals
with setting up a household in the community, and
rental assistance is available for those who

those who qualify. One of the main goals of MFP is
to enable the participant to direct the transition
process. In addition, six different service and
support packages are available to meet the
individual needs of those who transition to the
community.
Allied Community Resources continues its role as
a Fiscal Intermediary for this exciting program.
Allied processes the paperwork for new participants
sent to us from the Department of Social Services –
MFP Unit. We assist and train the participant in all
areas of becoming a Household Employer, hiring
Personal Care Assistants, and establishing an
Emergency PCA Back-Up system. We also process
applications for individuals who wish to be hired as
Personal Care Assistants for the MFP Program.
