ABI Basic Informational Session (Woodbridge)

This is an informational session on what a brain injury is and how it affects people.  It does not train you or certify you in anything and it is simply part of the application process.  If you are a private provider, the only services that require this session are Personal Care Assistant, Companion, Respite and Independent Living Skills Training.  If you are an agency employee, this session is required for Community Living Support Services, Companion, Respite and Independent Living Skills Training.

You must register for a session by calling Allied Community Resources (ACR) directly at 860-627-9500 or toll-free at 877-722-8833 and ask for the ABI Program Assistant.  You will be sent directions and a confirmation at that time.  If you have not registered properly through ACR, you will not be allowed entrance.

There will be no session on business holidays or in the event of inclement weather.  Please call Allied Community Resources if you have any questions regarding these sessions.

IT IS NECESSARY FOR YOU TO PRESENT A VALID ID FROM THE LIST BELOW IN ORDER TO GAIN ENTRANCE INTO THE SESSION. IF YOU DO NOT COME WITH AN ID FROM THE BELOW LIST YOU WILL NOT BE ALLOWED TO ATTEND.

  • DRIVER’S LICENSE (any state)
  • CT NON-DRIVER’S IDENTIFICATION CARD (CT only)
  • U.S. PASSPORT
  • U.S. NATURALIZATION CERTIFICATE
  • U.S. ARMED FORCES IDENTIFICATION CARD
  • U.S. IMMIGRATION CARD-RESIDENT ALIEN (Green Card)
